Report & written referral request

Your treatment time is for you

During a 50-minute appointment, Boris focuses his full attention on you: your symptoms, examination, treatment and personal guidance. Detailed reports and written requests commissioned separately are prepared outside this treatment time. This requires extra time and precision and is therefore billed separately, after discussing the scope of work and costs.

Routine record keeping is part of your care and does not incur a separate charge. Access to your records and the first copy of your own records remain free of charge.

Boris is regularly asked to provide physiotherapy information for personal injury cases or a written explanation for a GP or specialist. Detailed documentation requires extra time and precision: reviewing relevant information, carefully describing findings and answering the question clearly.

Report for a legal professional

€145

A commissioned physiotherapy report, for example for a personal injury case. Information is provided withapproved authorisation from the patient. We agree on the question, recipient and relevant information in advance.

Written referral request

€55

A written request with an explanation for your GP, doctor or specialist, based on the question discussed and available physiotherapy findings. The doctor or specialist independently assesses whether a referral or further investigation is appropriate.

This commissioned work falls outside regular treatment time and has a separate fee. We discuss the content, costs and expected turnaround time in advance. We share medical information only with the agreed recipient and with your consent.

From request to report

  1. Discuss your question.Contact us and explain who the information is intended for and which question needs to be answered.
  2. Agree on the scope of work.Boris discusses which information is available, what can be prepared, the costs and expected turnaround time. If more information or examination is needed, we discuss this in advance.
  3. Arrange the authorisation.Approved authorisation from the patient is required for a report to a legal professional. We agree on which information may be provided to which recipient.
  4. Receive the written explanation. The report is carefully prepared using relevant physiotherapy findings. The method of delivery is agreed in advance.

What does Boris describe?

Depending on the question agreed, the explanation may cover your symptoms, the examination performed, functional limitations and the course of treatment. A physiotherapy report is not an independent medical-legal expert assessment. A doctor or specialist decides independently on referrals or further investigation.

Requesting your own physiotherapy records

You have the right to access your own records and receive a first copy. This access and first copy are free of charge.Gathering and providing existing record information for this request is not billed as a paid report.

In addition to your records, are you requesting anew, separate report or written explanation? Preparing this requires extra time and care. Separate charges may apply; we discuss these with you in advance and ask for your agreement. Your right to a free first copy of your records remains unchanged.

A reasonable fee for administrative costs may apply to additional copies. Any charges are explained in advance.Read the Dutch government information on copies of medical records.
